Please know that if you choose to participate you are responsible for making sure you have adequate space and proper surfaces to dance and move. To avoid injury as you dance from home, it is important to listen to your body, set your own pace and modify as needed.

• Students must be familiar with using ZOOM.
• Students must be able to move on their own. If students cannot a caregiver should be in the room at all times.

By registering for this exercise class, I acknowledge and agree that participation in physical activity carries inherent risks, including the potential for injury. I voluntarily assume all risks associated with this class and release Wisconsin Parkinson Association, its affiliates, contractors, instructors, and staff from any liability related to my participation.

I understand it is my responsibility to consult with my medical provider or physician before beginning any new physical activity or exercise program.
