Waiver and Release of Liability: 
By registering for this event, I acknowledge and agree that participation in physical activity carries inherent risks, including the potential for injury. I voluntarily assume all risks associated with this class and release Wisconsin Parkinson Association, its affiliates, contractors, instructors, and staff from any liability related to my participation.

I understand it is my responsibility to consult with my medical provider or physician before beginning any new physical activity or exercise program.

By completing this registration, I confirm that I have read, understand, and agree to this waiver and release.

Photo and Video Notice

By registering for this event, I acknowledge that photographs or videos may be taken during the activity. These may be used by Wisconsin Parkinson Association (WPA) to promote programs, resources, and support for the Parkinson’s disease community through formats such as social media, newsletters, and other communications. If I prefer not to appear in photos or videos, I understand it is my responsibility to notify WPA staff or the instructor before the activity begins.
